COVID-19 Health Assessment & Safety Protocols for Visitors

To help ensure the safety of our visitors and team members, all visitors are expected to review and follow our safety protocols.

Prior to coming on-site, visitors MUST review the following COVID-19 Health Assessment confirming that they have not been exposed to and are not having symptoms of COVID-19. Anyone who is unable to agree to these statements is not permitted to come on-site and should contact the meeting host to reschedule or consider a virtual meeting if appropriate.

 

  1. Within the past 14 days I have not tested positive for COVID-19.
  2. In the past 14 days I have not experienced:
    1. Cough
    2. Shortness of breath
    3. Chills
    4. Muscle pain
    5. Headache
    6. Sore throat
    7. Chest pain or fatigue
    8. New loss of taste or smell
    9. Nasal congestion
    10. Runny nose
    11. Diarrhea
    12. Other symptoms
  3. In the past 24 hours I have felt well.
  4. In the past 14 days I have not been in close contact to a person who has been diagnosed with COVID-19.
  5. In the past 72 hours my temperature has not been above 100.4F.
  6. I don’t’ have any reason to believe I have COVID-19.

Upon arrival at our office, visitors must: 

  1. Call or email your meeting host or enter the glass foyer area and ring the white doorbell to inform them of your arrival.
  2. Electronically sign the COVID-19 Health Assessment Attestation via an iPad located at our reception desk.
  3. Maintain social distancing of at least six feet and wear a face covering at all times.
